If you have a general question or need to report an incident of bullying or harassment, please use the confidential form provided below. Your submission will be reviewed by: Team Leader Kelli Wilson or Lori McCoy.

When submitting your report, please ensure you include the following information:

• The Nature of the Incident: Clearly explain what occurred (e.g., general question, bullying, harassment).

• Date(s) of Incident: Specify the date(s) the incident took place.

• Parties Involved: List all individuals involved (names, if known, and their relation to the school).
